This is Your COCO Group Criti-Surgical Protect which has been issued by Us relying on the Information disclosed by You in Your Proposal for this Policy or its preceding Policy/Policies of which this is a Renewal. It contains details of what is covered, what is not covered, the conditions and the basis on which all claims will be settled. The proposal, Policy Schedule, Policy document and endorsements are part of the Policy. Your Policy is evidence of the contract of insurance.

C. Group III - Nervous System & Related Illness
11. Stroke Resulting in Permanent Symptoms
12. Permanent Paralysis of Limbs
13. Motor Neuron Disease with Permanent Symptoms
14. Coma of Specified Severity
15. Bacterial Meningitis
16. Benign Brain Tumor
17. Encephalitis
18. Multiple Sclerosis with Persisting Symptoms
19. Major Head Trauma
20. Progressive Supranuclear Palsy
21. Primary Parkinson's Disease
22. Multiple System Atrophy
